Important Forms
- Single Commuter Form (🔗)For students in the After Elementary class (ages 6+) who will travel to and from school alone, please complete this form to inform the school. This includes students who will be taking the bus independently.
- Authorized Pick-Up Persons/Health Information (This form is currently offline, please email the office with any changes.)
- Doctors Release Form (🔗)
If your child has been absent from school due to a contagious illness a Doctor's Release Form is required before they can return. The school will make the final decision regarding your child's return, even with a completed form. To be prepared for a smooth return, please bring the completed form from your physician, along with your child, to the office before going to the classroom. Contact the school office if you are unsure if a Doctor's Release form is needed.
School Absence
Please notify the school if your child will be absent.

🚌 - A direct notification to the bus is also necessary for students who use the school bus service.
Special Program Extended Care
For families enrolled in Special Programs who need extended care, Special Program Extended Care Tickets may be purchased at the school office. The tickets can be used for Early Drop-Off and Late Extended Care. Please fill out each ticket with your child's name and the date before handing them to the teacher.
Early Drop-Off:
- 8:00 - 9:30 a.m. - 1 Ticket
Late Extended Care:
- 3:00 - 4:00 p.m. - 1 Ticket
- 3:00 - 5:00 p.m. - 2 Tickets
- 3:00 - 6:00 p.m. - 3 Tickets
Notes:
- Tickets can only be used as whole units and cannot be split. For example, if you need extended care until 4:30 p.m., you will need to use 2 tickets.
- Regular Extended Care tickets cannot be used for Special Programs.
- All Extended Care reservations need to be made before 10:00 a.m. Reservations made after 10:00 a.m. will be charged double the ticket price.

Items to Bring
Please be sure that all items are labeled with your child's name.
- Hat (Kept at school and sent home on Friday)
- Water bottle (filled)
- Cup
- Utensils (Fork-and-Spoon or Chopsticks)
- Wet towel (for lunch)
- Extra clothes (kept inside backpacks unless needed)
- 3 diapers (only 2 years old class)
- Wet Wipes (only 2 years old class)
- Bib (only 2 years old class)
- Towel with a loop ( Size: No bigger than 35 cm in length from corner to corner)
- Inside shoes
Drop Off Time
- Our drop off time is from 9:30-10:00 a.m. every day
- If you have signed up for morning extended care, you may drop off anytime between 8:00 and 9:30
- If you arrive late (after 10:00 a.m.) please come to the 2nd floor office.
Pick Up Time
- Our two main Pick Up Times are 1:20-1:30pm and 2:45-3:00pm
- If you would like to arrange a special pick-up time for your child, please inform your teacher at the beginning of the day
In Case of Illness
- If your child’s temperature is 37.5 or higher or if your child exhibits any flu-like symptoms (coughing, sore throat, excessive coughing, body aches, fatigue) please keep them at home as we cannot allow them to enter the school. In case of absence, please fill out our school absence form: School Absence Form.
- If you child begins to exhibit any flu-like symptoms throughout the day, we will call you to pick them up. One parent must be available in case your child becomes ill.
- Use the Doctors Release Form if necessary.
Emergency Evacuation
Please follow the instructions below if a disaster occurs:
Case 1: When a big earthquake such as the Tohoku 3/11 earthquake occurs,please come to the designated evacuation shelter shown below.
During this time, the phones will most likely be out of service.
Our teachers will look after your children until you show up at the designated evacuation shelter of Bunkyo-ward:

School may evacuate or call parents for early pick-up in the case of structural damage or loss of services.

During an emergency, we may not have access to our computer systems and telephones. But, in the case that we have computer access, we will update the following locations. Please bookmark these locations before an emergency happens.
